Database not found issue

I downloaded the latest LiveZilla and installed on my Server with windows 2003,and MySQL. I have done this several times as one post said a reinstall might resolve the problem. It doesnt for me.
The server test completes fine .
I have major problems with the database configuration within the server profile, as it does not hold its settings. Whenever I re-visit the Database tab, it is always empty of non-pre-populated values.
I always successfully enter the values and test the connection, the service doesn't come up and they are blank when I return to the tab, and the database test says it is OK.

When I try to connect to the server administrator, it reports that it cannot connect to the database.

Embedding the live chat button on a web page also reports the service offline.
I have been trying every possible combination to get this to work, with still teh same problem, including reading every post on this issue. Can anyone help?
I have seen a similar post here suggesting IUSR but this has not worked for me.
If anyone can assist, or if I can pay someone to sort this issue , please leave a message, or PM me.
This is really urgent now.

Hi it is still not working. :(
I am running php 5.3.5 and mysql 5.1.57, PHP is not in safe mode.I have just ftp'd every permission as your list and these are the same as set on the server.
The web based part is working. But the main web software cannot log into the database.
From the admin section, it writes the login details to the database but login fails when we log in to it- see screengrab from the server.
Again if anyone offers an installation service please let me know.I would love to fix this by monday as the site is live.
